The goal is simple: help families take care of appropriate routine health needs without making the process more complicated than it needs to be.

Have a form that needs a medical provider?
School requirements can create last-minute headaches. Owens Elite can help determine what evaluation or documentation is necessary to appropriately complete your child's form.
Some forms require more than a signature.
A medical form can only be completed when the provider has sufficient medical information to accurately and appropriately certify what the form requires.
Depending on the form, your child may need a physical examination, review of medical records, vaccination documentation, testing or another medical evaluation before the form can be completed.
Start by understanding what's happening.
Children can experience allergy symptoms for many reasons. Evaluation begins with your child's symptoms, history and potential triggers.
When clinically appropriate, additional testing, treatment recommendations or referral to an allergy specialist may be discussed.
Pricing based on the service your child needs.
Because school forms, physicals, virtual evaluations and mobile appointments require different levels of care, pricing varies by service.
You'll know the applicable visit or service fee before care is provided.

What should I have ready?
Questions for parents.
Can you complete my child's school form without an appointment?
It depends on what the form requires and whether Owens Elite has sufficient current medical information to appropriately complete it. Some forms require a physical examination, record review or additional evaluation before they can be signed.
Can a sports physical be completed virtually?
A physical examination generally requires an appropriate in-person assessment. The required visit format will depend on the examination and documentation requested.
Do you provide allergy testing?
Owens Elite can evaluate appropriate allergy concerns and discuss whether testing may be useful. The type of testing needed depends on the child's symptoms and medical history, and some children may require testing or evaluation through an outside laboratory or allergy specialist.
Can you prescribe medication for my child?
Prescription treatment may be provided following an appropriate medical evaluation when clinically and legally appropriate. A specific prescription is never guaranteed.
Can you provide a mobile visit at our home?
Select mobile services may be available on a case-by-case basis depending on the type of care needed, geographic location and scheduling availability.
Does a parent need to participate in the appointment?
A parent or legal guardian should be available to provide consent and participate in the child's care when required. Additional documentation may be requested depending on the circumstances.
What if my child is seriously ill or injured?
Owens Elite children's services are not emergency services. For a medical emergency or serious injury, call 911 or seek immediate evaluation at an appropriate emergency facility.
